Before your camp session start date, you will receive an e-mail with forms that will be specific to your session. These forms should be completed, as applicable, and brought with to camp.

Required for all campers unless the camper is attending with an adult for Family Camps.

Required for camps lasting four days or more and council-led day camp at Edith Mayo; attach a copy of your/her insurance card.

Required for troop programs, and should be kept in the troop leader’s records.

Required for girls with ongoing medical conditions who are attending a camp lasting four days or more.

Required for all camps and must be filled out for the 7-10 days leading up to your camper’s camp session.
Instructions for Resident Camps
Complete the forms online OR bring all printed completed forms to camp. Girls will not be able to stay at camp without having completed the online form or having their printed forms in hand.
Instructions for Troop Camps
Troop leaders should collect a health history form for each camper. With the exception of a few sessions, you will retain these forms and the campers’ medications during their stay. If your troop members complete the online form, they can forward you the copy attached to their confirmation email for your records.
Instructions for Family Camps
Campers attending with their adult do not need fill out a health history form. Please bring any needed medications or allergy medicines, which can be kept with the adult.
